Forms to be filled out prior to your first appointment can be found at the bottom of this page. You may also choose to arrive 15 minutes early in order to fill out forms before your initial evaluation. Please notify us via email if you would like to have forms prepared for you.
PLEASE READ INSTRUCTIONS FOR SCHEDULING APPOINTMENTS CAREFULLY PRIOR TO BOOKING:
Click the "Book Appointment" button above to schedule, then create an account using your full name and use that same account every time you schedule an appointment. Do not choose the option to "skip the login process"!
All patients seen in the state of Illinois need a valid diagnosis in the form of a prescription from a medical doctor, osteopath, dentist or podiatrist. This is the case even if you are being seen on a self-pay basis. You may simply have your physician or other referring practitioner fax a prescription/referral to 847-492-0321 or bring it with you. You may have an evaluation (first appointment) without a referral.
Blue Cross and Blue Shield policyholders: please do not schedule appointments for more than 4 consecutive months after the date of your initial evaluation, as your policy does not cover more for any given diagnosis. If you're an existing patient, please contact me before scheduling if you are not sure how long you've been seen under your current diagnosis. This is NOT the same things as your yearly allowance, and you should be able to have therapy at another time during the same year. And yes, as we all know, using health insurance is complicated, so feel free to contact me for clarification of this or any other point.
Medicare requires that you have a referral from an MD or DO (medical doctor or osteopath) written no longer than 3 months prior to your evaluation.
Medicare does allow you to be evaluated without a referral.

If you need to cancel, please do so as soon as possible. Please be considerate and do not wait until right before the cancellation window (24 hrs) kicks in, as cancellations this close to the appointment time are hard to fill. Out of respect for your fellow patients and your therapist, cancel early in order to give patients on the waiting list an opportunity to book the appointment slot.
You can always check the schedule for the next available appointment time. If you wish to be notified of cancellations, please let me know. If you are an new
patient scheduling your first appointment, do consider scheduling the appointments you will need for a month or two as you otherwise may face a wait after your initial evaluation before your next appointment.
Cancellation policy: appointments must be cancelled 24 business hours (= Monday appointments must be cancelled by the preceding Friday) before the scheduled appointment time. Late cancellations and no-shows, no matter what the reason for them, will lead to a $145 (the cost of the appointment) cancellation fee.
